  • Lifestyle or athletic information relevant to your Analysis

3. Personal Identifiers

  • Payment information (processed through third-party vendors; we do not store credit card details)

4. Authorization Forms

If you complete our HIPAA-style authorization, we collect:

  • Names and contact details of clinicians, coaches, or trainers you authorize

  • Scope of information you permit SwingDx to share

  • Your signature and date

B. Information We Collect Automatically

When you visit our website, we may collect:

  • IP address

  • Device type

  • Browser type

  • Pages viewed

  • Time spent on the site

  • Cookies or similar tracking technologies

These are used for website functionality and optimization.

C. Information from Third Parties

If you authorize us to share your report, your designated clinicians or coaches may provide feedback, questions, or clarifications. That information becomes part of your record only if you share it with us voluntarily.

  • Tailor filming instructions

  • Interpret your swing using contextual health narrative

C. To Share Information You Explicitly Authorize

If you complete the HIPAA-style release, we will share your report or designated information only with the parties you name.

  • Enhancing analytical models

  • Developing educational materials (with personal identifiers removed)

  • Improving workflow and consistency

We will never sell your data or use it for targeted advertising.
