Your intake form provides the essential context behind your swing—your personal narrative, pain points, past injuries, mobility limitations, goals, and anything else that helps explain why you move the way you do. Once you complete the form, we use that information to tailor the filming instructions we send you and to ensure your AI motion-capture analysis is interpreted correctly. When paired with your swing data, this context allows the AI to deliver a report that’s not only accurate, but also clinically relevant and highly personal. This is also where you define exactly who is authorized to receive your report, through the HIPAA-compliant “Share Information” section where you can list your clinician and any performance coaches.

Our system then analyzes this data against performance and anatomical benchmarks, connecting movement patterns to identify physical limitations and risk factors.
After purchasing, you’ll receive a link to complete your SwingDx Intake Form. As part of the form, you’ll share a brief personal narrative describing your swing, your goals, and any physical challenges you’re experiencing. Once submitted, we’ll send you instructions to film your swing using our AI golf swing motion-capture system. Your data is then analyzed and transformed into a clinical-grade performance report, which is shared only with the clinicians and performance coaches you list in the HIPAA “Share Information” section of your intake form.
